New Student Compliance Requirements
Admitted students must meet all new student compliance requirements prior to starting the program as well as returning student requirements for programs that are two years in length. Applicants should not start this process until after receiving an offer of admission.
NOTE: Failure to meet any of these requirements can lead to my offer of admission being rescinded, a delay in my clinical training, or inability to finish the program.
Students who are new to IU School of Medicine's Health Professions Programs are required to document compliance in the following areas.
Please note: Students in the Respiratory Therapy Program complete requirements set forth by the Indiana Respiratory Therapy Consortium and may disregard these instructions.
Please note: Students in the Histotechnology Program that are employees of their clinical site may disregard these instructions.
Background Check and Drug Screen
Within 10 days of receiving your acceptance notification you should contact
Castle Branch to register for a drug screen and background check. Please use package code IC91 rate of $95.75. If you do not receive your results, particularly the drug screen result as this takes more time, from the company within 8-10 days, please contact Castle Branch immediately. Our office will see the results online. You do not need to contact us regarding these results. If there is a problem, we will contact you.
Proof of Health Insurance
Submit proof of health insurance no later than June 1. Please email this form along with a copy of the front and back of your health insurance card to email@example.com. The university does not provide health insurance.
Immunizations & Annual Health Evaluation (Physical)
Provide proof of a health evaluation (physical). This
form should be completed and signed by IUPUI Campus health no later than June 1. If you will be using an outside provider, please use the same form and email it to Campus Health at firstname.lastname@example.org. This form should not be submitted to our office as this violates HIPAA regulations. You should have also submitted your immunization information upon acceptance into IUPUI. If this has not yet been done you will also need to submit proof to Campus health.
TB Skin Test
We require an annual TB skin test which can be completed at the IUPUI Student Health Clinic for $17. If you choose to have your TB test done somewhere other than Campus Health, please submit proof to
email@example.com. You are required to complete the 2 step process as a new student to HPP. Please refer to https://studentaffairs.iupui.edu/health/medical/index.html with questions.
Please follow these timelines for the TB testing so that your test is effective for the entire first year of your program. The dates indicated below are the dates you should follow.
All students must follow Indiana University guidance related to COVID-19. For more details, please visit https://www.iu.edu/covid/index.html/.
We require an annual flu shot which should be completed in the fall before November 1st. Please watch the Student Health
website for availability. If you obtain a shot at an outside vendor please provide documentation to firstname.lastname@example.org.
All A.S. Radiography students must have fit testing done at Campus Health on the IUPUI campus no later than June 1. Please refer to your program director for more information about this.
All clinical Medical Imaging Technology students are required to be fit tested before the fall clinical rotation. Please refer to your program director for more information about this. If your fit testing is done somewhere other than Campus Health, please provide proof to campus health at

All A.S. Paramedic Science students must have fit testing done. Please contact program director, Leon Bell, for instructions on how to complete the fit testing.
