​​​​​​​​Returning Student Compliance Requirements


NOTE: Failure to meet any of these requirements ​can lead to a delay in my clinical training or inability to finish the program. 


Students returning* for a second year in any of the IU School of Medicine Health Professions Programs are required to document compliance in the following areas:

  • Background Check & Drug Screen+
  • Health Insurance
  • Health Screen
  • Tuberculosis Skin Test (TST)
  • Flu Shot
  • Fit testing^

Students who have recently had background check, drug screen, or health screen processes completed based on a recent job opportunity may be allowed to use that documentation to meet this requirement.

Students in the Respiratory Therapy Program complete requirements set forth by the Indiana Respiratory Therapy Consortium.

*This also includes students who have failed to meet all curricular requirements and have extended their time in the program beyond the expected graduation date.

+The requirement to complete a drug screen is not a school policy but meets requirements as outlined in the school's clinical affiliation agreements with our various clinical partners.

^ This is a requirement for Radiography, Paramedic Science, and clinical Medical Imaging Technology students only.

Background Check & Drug Screen

Prior to June 1, you must contact Castle Branch to register for a drug screen and background re-check. Enter package code number IC91re for the re-test rate of $60. If you do not receive your results, particularly the drug screen result as this takes more time, from the company in a timely fashion, please contact Castle Branch immediately. Our office will see the results so you do not need to contact us regarding the results. If there is a problem we will contact you.

 

MIT ONLY: Returning MIT Students who have internships with a clinical site can provide proof of employment to exclude them from this requirement. You do need to keep in mind that if we need proof that you have completed either or both of these requirements you will need to provide it to us from your employer. Please send proof of employment to askhpp@iupui.edu no later than June 1. If you are not employed in this manner, please proceed with the background check and drug screen as instructed above.


Health Insurance


Submit proof of health insurance no later than June 1. Please email this form​ along with a copy of the front and back of your health insurance card to askhpp@iupui.edu or mail to Health Professions Programs. The university does not provide health insurance

 

Health Screen

                                

Provide proof of a health evaluation(physical). This form should be completed and submitted to IUPUI Campus health no later than June 1. If you use an outside provider, please use the same form and email it to healthsv@iupui.edu. This form should not be submitted to our office as this violates HIPAA regulations. Your immunization information should already be in the Campus Health system.

You do need to keep in mind that if we need proof that you have completed this requirement you will need to provide it to us from your employer. Please send proof of employment to askhpp@iupui.edu no later than June 1. If you are not employed in this manner, please proceed with the health screen as instructed above.
 

Tuberculosis Skin Test (TST)

                                            

Complete an annual TB skin test at the IUPUI Student Health Clinic, which costs $17.  You are only required to take the single step test. Please refer to this website​ for any additional questions. 


The TB test is effective for one year. In order to remain compliant during the duration of your time in the program, please plan to receive your TB test near the same time that you received it last year. You MUST have it completed before the first day of classes.

COVID-19


All students must follow Indiana University guidance related to COVID-19. For more details, please visit https://www.iu.edu/covid/index.html/.


Flu Shot

                                                           

All IUSM faculty, staff, house staff and students who provide clinical care to Indiana University Health or Eskenazi Health patients, perform clinical research at IU Health or Eskenazi Health, or whose primary office is in an IU Health or Eskenazi hospital, are required to receive a flu vaccine. IUPUI Campus Health will offer free flu shots this flu season. Individuals may get a flu shot by visiting Campus Health in Coleman Hall, Suite 100, or a Campus Health outreach flu vaccination clinic. No appointment is necessary, but individuals must present a valid university photo ID to get a flu shot. Students will be asked to present a health insurance card, but will be able to receive a free flu shot regardless of whether or not they have health insurance. IUPUI Campus Health has more details and contact information.

Fit Testing

All Radiography, Paramedic Science, and clinical Medical Imaging Technology students are required to be fit tested before the fall clinical rotation. Please refer to your program director for more information about this. If your fit testing is done somewhere other than Campus Health, please provide proof to Campus Health at healthsv@iupui.edu no later than June 1.
